Licensed Vocational Nurse (LVN) –
Home Health (Multiple Openings)
Locations:
• Northeast Houston, TX

About the Role: (W2 employment.)
Our client is seeking experienced and dependable Licensed Vocational Nurses (LVNs) to join
their growing home health team. Ideal candidates will bring strong clinical skills, professionalism,
and a collaborative, patient-centered approach. This role involves providing skilled in-home
nursing services, including assessments, wound care, and specialized procedures. Candidates
must be comfortable traveling within a 40-mile radius of their home address.
Key Responsibilities:
● Perform comprehensive body assessments and document findings within required
timeframes (48 hours).
● Provide wound care, including wound VAC management.
● Conduct phlebotomy (blood draws) as ordered.
● Perform foley catheter insertion and care, PICC line care, and tracheostomy care.
● Deliver patient-centered care under the supervision of an RN or Case Manager.
● Communicate effectively with patients, families, and the interdisciplinary team.
● Maintain timely, accurate documentation compliant with agency and regulatory
guidelines.
● Adhere to professional appearance standards and company policies.
Qualifications:
● Active LVN license in the state of Texas.
● Minimum 1 year of clinical experience, preferably in home health or skilled nursing.
● Strong clinical judgment and assessment capabilities.
● Reliable transportation and willingness to travel up to 40 miles.
● Proficiency in wound care, PICC line management, tracheostomy care, and catheter
care.
● Ability to work independently while maintaining communication with the care team.
